This is a FIRST SHOT Clinic ONLY! Do not register if you need a 2nd shot.

ELIGIBILITY

* If you have received any vaccine in the 14 days prior to your COVID vaccine appointment, you will not be eligible to receive your COVID vaccine. You must wait at least 14 days between vaccines.

* Please do not sign up for a first dose vaccine if you recently tested positive for COVID-19. The CDC recommends that as reinfection is uncommon in the 90 days after initial infection, a vaccination should be deferred for at least 90 days from the date of a positive test, especially if treated with monoclonal antibodies or convalescent plasma.

To be eligible for vaccine, you must be a Florida resident+ and fall under the Governor’s Executive Order 21-79. "Effective April 5, all Floridians shall be eligible to receive any COVID-19 vaccine as prescribed by the Food and Drug Administration." For Pfizer, recipients may be 16 years of age or older.

Florida Residency

+ On January 21, 2021, State Surgeon General Dr. Scott Rivkees signed a Public Health Advisory prioritizing Florida residents for COVID-19 vaccinations given in Florida. Prior to the first dose of COVID-19 vaccine, every recipient must provide a Florida Driver’s License, State ID OR two other forms of documentation, demonstrating Florida residency consistent with the criteria set forth in 381.986(5)(b), Fla. State.; or proof recipient is present in Florida for the purpose of providing health care services involving direct contact with patients. Before registering, please check the 2nd shot due date to make sure you will be available.

It will be 21 days from the date of your first shot at the same location and time block that you select today. The volume of people being vaccinated at the clinics does not allow for accommodation for individual scheduling requests, except in extreme circumstances, such as a hospitalization or contracting COVID.

TO REGISTER

Check your entries for accuracy!

  1. Select your preferred time slot. Make sure to scroll down to view all of your date/time options.
  2. You can only add one ticket, but can reuse the link to add other people.
  3. For each ticket, enter the name of the person as it appears on their ID. Make sure that you have entered different names on each ticket and when complete, press Register. Tickets with the same name will be deleted by the system as duplicates.
  4. Select View Tickets, and click on the order.
  5. Make sure all information is accurate. If it is not accurate, select edit and make necessary changes.
  6. When finished, select Print Tickets. This will create a document that will need to be saved and then sent to a printer. If you cannot print, you can open the Eventbrite app or show the ticket from your email on your smartphone.

Download and complete the COVID-19 Vaccine Screening and Consent Form to bring with you to the appointment. If you are not able to print the consent form, you will be provided a blank one to complete on site.

How do I change the name on my ticket?

  1. Log into your Eventbrite account and select Tickets in the menu under your email in the upper right corner. The order will always be associated with the email address you entered during registration. If you made a typo, changing the email address on your ticket will not make it show in the Eventbrite App. In this situation, you’ll need to use the order confirmation sent to your email address and email the organizer to make the change to your ticket.
  2. Click the name of the event to select your order.
  3. Click "Edit".
  4. Click “Edit” on the individual ticket and change the name.

How do I cancel my ticket?

Please cancel your ticket as soon as possible and not later than 24 hours prior to the event so that others can take advantage of the opening.

  1. Go to "Tickets," select your order and choose "Cancel Order".
  2. Go to your order. Log in to your Eventbrite account, then click your order on the Tickets page.
  3. Choose "Cancel Order".
  4. Select "Cancel Order" (next to your order details). Then choose “Yes, cancel this order".
  5. Check for an email from Eventbrite. The attendee and event organizer receive an email to confirm cancellation.

DAY OF YOUR APPOINTMENT

If you do not feel well, please do not come for your appointment and contact your doctor. COVID infection symptoms are many and we do not wish to have anyone exposed at a clinic. Vaccination needs to be deferred until you recover from the illness or have met the criteria to leave quarantine. We will be happy to reschedule you for a future clinic.

3. This event is a walk-through clinic that requires walking and standing in line to get to the vaccination point. Chairs are available throughout the waiting area and a wheelchairs MAY be available for those who need them. Lines may be long; please plan accordingly.

4. A 15 to 30-minute observation period is recommended after your vaccine is administered.

5. You must wear a mask and maintain physical distance of at least 6 feet at all times.

Accepted forms of identification:

  • To verify age and address - Driver's License, passport or other government issued picture ID with address and date of birth.

Second Dose

Once vaccinated, you will receive a Vaccine Record Card that indicates the appointment for your 2nd shot on the back, 21 days after your first shot. SAVE THIS CARD. You may want to take a picture of both sides to save for future reference.

The volume of people being vaccinated limits the ability to reschedule 2nd shot appointments to accommodate individual needs. Please make certain you can make your 2nd shot appointment time before completing this registration. Plan to attend at the same location and time block 21 days after your first dose. Bring your Vaccine Record Card, another completed vaccination consent form, and an Eventbrite ticket, if you get one.

Staff must manually enter 2nd appointments from each event (up to 2,000 for each day). It takes time for everyone to receive an Eventbrite ticket. Incorrect emails create challenges when attempting to send second appointments. If you came with someone else for your first shot and they do not receive a ticket, THERE IS NO NEED TO EMAIL US TO REQUEST THAT THEY GET AN INVITATION. They can be admitted with their Vaccine Record Card.

REMEMBER - If you have received any vaccine in the 14 days prior to your COVID vaccine appointment, you will not be eligible to receive your 2nd shot.
